Dear SELECT Math Participant:

Thank you for taking the time to complete this follow-up questionnaire. The SELECT Math project team is interested in knowing what activities you have been involved in since the beginning of this initiative and what you may have learned through your experience in SELECT Math. Your responses to this questionnaire will help shape future professional development training and support models. Your responses will be anonymous. However, we do require that you provide your first and last name in order for us to track your experience throughout the SELECT Math project.

DIRECTIONS: Please complete this electronic questionnaire as fully as possible. The questionnaire consists of multiple choice and short answer questions. There are two questionnaires below. The first is for Math Coaches. If you have identified yourself as a Math Coach, please click on that post-course survey. The second questionnaire is for all other positions. If you have identified yourself as a teacher, resource teacher, administrator, etc, please click on that post-course survey link.
